File Form 1099-SA, Distributions From an HSA, Archer
MSA, or Medicare Advantage MSA, to report distributions made from HSA, Archer MSA,
or Medicare Advantage MSA (MA MSA). The distribution may have been paid directly to
a medical service provider or to the account holder. A separate return must be filed
for each plan type.
About Our
Software...
Copy A has the look and feel of a normal sheet of
paper printed with a special red ink. If you submit Copy A by
mail to the IRS, it must be submitted on their page with their
special ink.
Copy B and Copy C can be printed on ordinary paper
or on the tissue-like paper the IRS provides.
With our software, you:

information on the screen as it would be typed on the form
itself.

Detach the
perforated edge from Copy A and then insert into your
printer.

Print your
data neatly on top of the form.

Data entered
may be stored/saved anywhere on your hard drive or to external
diskettes for quick retrieval.
Who to
furnish a copy?
Form consists of 3-pages where Copy A is sent to the Internal
Revenue Service, Copy B is given to the Transferor and Copy C is
transmitted to the Filer and/or State.
Due Dates
Furnish Copy B of this form to the
recipient by January 31.
File Copy A of this form with the IRS by February 2. If you file electronically, the
due date is April 2, 2007.
