What is Form 1095-C?
Beginning in 2016, employers must file Forms 1095-C with the IRS to report information about the offers of health coverage
made to full-time employees during the previous calendar year and provide copies of Forms 1095-C to those employees. Copies of
Forms 1095-B and/or 1095-C should be provided to you early in the calendar year for the previous year.
You will receive a Form 1095-C if you were a full-time employee (as defined by the ACA) for all or some months of the prior
calendar year. You receive a copy of the Form 1095-C so you know what information has been reported to the IRS about the offer
of health coverage made to you and your family.
