Each column has to be read in the correct sequence. The 1st column on your data identifies whether this information return is Void. The 2nd column has a value for Corrected and so forth following in the order below for the 1099-DIV:

DIV_1B // qualified dividends
DIV_2A // total capital gain distr.
DIV_2B // unrecap. sec 1250 gain
DIV_2C // section 1202 gain
DIV_2D // collectibles (28%) gain
DIV_3 // nondividend distributions
DIV_4 // federal income tax withheld
DIV_5 // investments expenses
DIV_6 // foreign tax paid
DIV_7 // foreign country or us possession
DIV_8 // cash liquidation distributions
DIV_9 // noncash liquidation distributions

Type of TIN is taken from the TIN itself. If the TIN contains one dash (-), then the TIN type is EIN. If the TIN contains two dashes, then the TIN type is an SSN. Otherwise the TIN is identified as unknown.
