Import 8955-SSA Data

This page shows the order of data used for importing data into IRS Form 8955-SSA software.

Each column has to be read in the correct sequence. The 1st column on your data is the Entry Code, the 2nd column is the Social Security Number, then First Name of Participant, Middle Initial and Last Name of the Participant following in the order below for the 8955-SSA:

(a) Entry_Code
(A, B, C, or D)
(b) Social_Security_Number
(c) First Name of Participant
(c) Middle Initial of Participant
(c) Last Name of Participant
Check (1 or 0)
(d) Type of Annuity (A, B, C, D, E, F, G, M)
(e) Payment Frequency (A, B, C, D, E, M)
(f) Defined Benefit Plan
(g) Defined Contribution Plan
(h) Previous Sponsor's EIN
(i) Previous Plan Number

Type of TIN is taken from the TIN itself. If the TIN contains one dash (-), then the TIN type is an EIN. If the TIN contains two dashes, then the TIN type is a SSN. Otherwise the TIN is identified as unknown.
