Import 1095-A Data
This page shows the order of data
used for importing data into IRS Form 1095-A software.
The excel file you develop (as well as the sample excel file here) follows
the form itself. The first column identifies whether
this information return is Corrected. Enter a 1 or YES to put a check in
that box or enter a 0 or leave blank to leave unchecked. The 2nd column has
the Marketplace Identifiier, then Markerplace-assigned policy number and so
on, following Form 1095-A downward.
The order is below:
Corrected (0 or 1)
1 Marketplace identifier
2 Marketplace-assigned policy number
3 Policy issuer's name
4 Recipient's name
5 Recipient's SSN
6
Recipient's date of birth
7 Recipient's spouse's name
8 Recipient's
spouse's SSN
9 Recipient's spouses date of birth
10 Policy start date
11 Policy termination date
12 Street address
13 City or town
14
State or province
15 Country and ZIP or foreign postal code
Now list the covered individual name, SSN, Date of Birth, Start Date and
Termination Date and stack each covered individual in the excel file.
A Covered Indivudal Name
B Covered Individual SSN
C Covered
Individual Date of Birth
D Covered Individual State Date
E Covered
Individual Termination Date
The form provides space for 5 rows of
data so if you have more than 5 covered individuals in that household, the
software will import in the first 5 covered individuals, create a new Form
1095-A and then start filling the 6th covered individual on that new Form
1095-A.
Then list the remaining fields:
21A Monthly Premium Amount
21B Monthly Premium Amount of Second Lowest
Cost Silver Plan (SLCSP)
21C Monthly Advance Payment of Premium Tax
Credit
22A Monthly Premium Amount
22B Monthly Premium Amount of
Second Lowest Cost Silver Plan (SLCSP)
22C Monthly Advance Payment of
Premium Tax Credit
List out the 12 months. You do not have to
list the totals at the end because the software will calculate the totals.
