IRS Form 1095-A is sent to people and family members who enrolled in health insurance coverage through a Health Insurance Marketplace. Form 1095-A provides information needed to complete Form 8962, Premium Tax Credit (PTC) which is filed with personal tax returns.
Form 1095-A shows recipients the dates of coverage, total amount of the monthly premiums for insurance plans, the second lowest cost silver plan premium that may be used to determine the amount of premium tax credit, and the amount of advance payments of the premium tax credit.
We developed 1095-A software. Our 1095-A service bureau lets you send the data to use in Excel. We will import from excel and convert to PDF for you to review. Upon approval, we can print and mail the 1095-A forms.
