Each column has to be read in the correct sequence. The 1st column on your data is the Establishment Name, the 2nd column is the Street Address, then City, State and Zip for the Establishment following in the order below for the 8027:

Type of TIN is taken from the TIN itself. If the TIN contains one dash (-), then the TIN type is EIN. If the TIN contains two dashes, then the TIN type is an SSN. Otherwise the TIN is identified as unknown.
